July 2016, Highways England (HE) advised:
The form has been revised slightly and the latest version is attached. It is designed to capture all the pertinent information we require to enable an investigation to be undertaken by our service providers. I appreciate that lengthy forms do not suit everyone but, unfortunately, this is the only format we have available to us at the moment. However there is nothing to stop a claimant seeking assistance from a family member or friend to complete the form.

Claim for damage to your vehicle
You may be able to claim compensation if your vehicle has been damaged by a road. The organisation you contact depends on where the road is and the type of road.
You can’t claim compensation if debris from another vehicle caused the damage. Contact your insurer instead.
How to make a claim
Contact the organisation responsible to tell them:
- what the damage was
- why you think they are responsible
- the specific location where the damage took place – the road name and the nearest marker post number or feature which identifies the part of the road you were on
- the date and time the damage was caused
Trespassing on a motorway is a criminal offence. Don’t visit or try to photograph the location where your vehicle was damaged.

Other English roads – Report the problem to the council that manages the road then contact them for compensation.
Contact Traffic Wales for most A-roads and motorways. You can check which roads Traffic Wales manages. For all other roads, report the problem to the council that manages the road then contact them for compensation.
Apply online to Transport NI to claim compensation for all types of roads.
